Would you like to join QCDC for the 2021-22 season? Please fill this form out (one per dancer) and submit by Wednesday, June 9th. Once the form is submitted, you will receive a follow-up email explaining the audition process and any other requested information needed. A non refundable deposit of $50.00 is due by June 13th. This deposit will be applied to your final invoice at the end of the 2021 season. Please submit this deposit by Venmo @JKswart or by check to Queen City Dance Academy.
